New Employees
Welcome to Pulmonx! As a new employee, you are eligible for group benefits on your date of hire. Your initial benefit enrollment period is 30 days from your date of hire. Complete this checklist, read the Benefits Guide, and use online tools and resources to help you make decisions.
Understand how your health plan works. Learn more about the plans by reviewing the plan details in the 2025 Benefits Guide.
Consider your costs. Review coverage costs by accessing the Benefit Cost Sheet. You may also want to review the Benefits Guide where you will find the cost of the benefits as you review your options. During the election process in UKG the cost of the benefits you choose will appear while onboarding.
Add your beneficiary designations to Life and AD&D insurance coverage.
Take advantage of the Flexible Spending Accounts, or FSAs for short. We offer our employees a variety of ways to maximize your benefits by using Tax Advantaged Accounts. These spending accounts are used to pay for health care with a general purpose or limited purpose Health Spending Account, Dependent Care and Commuter Benefits (transit and parking) that allows you to set aside before-tax money to help pay for eligible expenses. If you participate in an Health Savings Account (HSA), the Company will also make an annual contribution to this account. Click here to Learn More
If you enroll in a High Deductible Health Plan (HDHP), you must also select the Health Savings Account (HSA) in UKG to ensure you receive the Pulmonx contribution into your account. Please note, you will be asked to select an employee contribution as well. If you do not wish to contribute any additional money to your HSA then you may select $0 to only receive the contribution from Pulmonx.
Access ADP to enroll for benefits within 30 days from your date of hire.
Add your dependents’ information in ADP. Make sure your dependent's information is accurate and complete. In addition to their name(s), be prepared to enter birthdates, social security numbers (required for reporting), and addresses if they do not reside with you.
Enroll in your 401(k) account and review other benefits. Note: The 401(k) plan is managed through Empower Retirement directly, not in ADP. You will be emailed a separate 401(k) communication. Learn more.
Download a copy of the Pulmonx Wallet Card for access to benefit contacts on hand. You may also request a laminated Wallet Card from Human Resources.
Within 30 days of your date of hire...
Within 30 days of your date of hire, you must log into ADP to:

Determine which benefits you may want to elect for you and your eligible dependents. Coverage is effective on your date of hire.

If you do not enroll...
If you do not enroll within 30 days of starting as a new hire, you will not be able to enroll for:

Health care coverage (medical, dental/vision)

​You will be required to wait until the next Open Enrollment period, unless you experience a qualified status change.

The remainder of the automatically enrolled options will be paid in full by the Company ​

​​Basic Life Insurance, AD&D Insurance, Short-Term Disability, Long-Term Disability and Employee Assistance Program.

Log in to: ADP within 30 days of your date of hire. You should receive a login email from ADP to start your onboarding process.
