Cigna Medical Plans

Pulmonx offers a variety of medical plan options through Cigna. All plans cover in-network preventive care at 100%, prescription drugs, and include an annual limit on your expenses.

 

​The key differences between plans are:

  • what you pay for the plan

  • what you pay when you get care

  • how out-of-network care is covered, and

  • your annual maximum cost for care (out-of-pocket maximum.)​

 

Cigna OA-20
This plan
 provides coverage nationwide at in the network level only. The Cigna OA-20 plan will not provide out-of-network coverage. This plan does not require you to elect a Primary Care Physician (PCP) and you may self-refer when seeking care from a specialist. 


Pulmonx also provides a choice of three Provider Organization (PPO) plan options. A PPO plan allows you to choose where you can go for care without requiring a referral from your PCP. You may also choose to visit providers participating in the network or out-of-network. The three plan options are:

Cigna PPO 20-350 
This plan has a $20 primary care provider office copay ($30 copay for specialty providers), with a $350 individual calendar year deductible and up to $750 family calendar year maximum deductible. There is also maximum out of pocket protection of $3,000/individual and up to $6,000/family.


Cigna PPO 30-750
This plan has a $30 primary care provider office copay ($40 copay for specialty providers), with a
$750 individual calendar year deductible and up to $2,250 family calendar year maximum deductible. There is also maximum out of pocket protection of $5,250/individual and up to $10,500/family.


Cigna HDHP PPO $1650
The Cigna HDHP PPO $1,650 plan is HSA-qualified, which gives you access to a tax-advantaged Health Savings Account (HSA). The HSA is a bank account that allows you to set aside pre-tax dollars to help you pay for eligible health care expenses. â€‹

Cigna Medical ID Cards

​

Cigna uses digital ID cards. You may download your ID card by visiting the Cigna member portal, myCigna or via myCigna mobile app. Cigna ID cards will not be mailed unless member requests via myCigna portal or by contacting Cigna Member Services at 866-494-2111.
