Kaiser Medical Plans
Kaiser is a Health Maintenance Organization (HMO) that provides comprehensive medical services, including patient care, hospitalization, supplies, and prescription drugs through its own network of doctors, hospitals, and affiliated facilities.

Both plans cover in-network preventive care at 100%, prescription
drugs, and include an annual limit on your expenses. The differences are:

what you pay for the plan,

what you pay when you get care, and

your annual maximum cost for care (out-of-pocket maximum).

Pulmonx offers two Kaiser plans:
Kaiser HDHP HMO (HSA-Qualified**)
Coverage Details

Network Requirement: Kaiser covers your medical expenses only if you use a Kaiser provider or facility.

Emergency Coverage: You are also covered for life-threatening emergencies outside of a Kaiser service area.

Primary Care Physician (PCP): Once enrolled, you must select a PCP to manage your care and refer you to specialists.

Location: These plans are available to employees who live in California.
** The Kaiser HDHP HMO plan is HSA-qualified, providing access to a tax-advantaged Health Savings Account (HSA).

Kaiser ID Cards

Kaiser ID cards will be mailed to new enrollees. You may access a copy of your ID card via Kaiser member account by visiting www.kp.org or via Kaiser mobile app. You may also contact Kaiser Member Services at 800-464-4000.
